SIVUH Privacy Notice
The SIVUH privacy notice sets out how we process all personal information that we generate and hold in the course of providing healthcare services. It explains what personal information we collect on data subjects as data controller, how we use it, who we share it with and the security in place to protect it. It also sets out the privacy rights that data subjects have under General Data Protection Regulation (GDPR) and Irish data protection legislation.
Request for Records
How to make a Data Protection request:
All requests for records are processed by the FOI Office. You must apply in writing and simply refer to the Data Protection Act 2018 or GDPR 2018. You must provide sufficient information in order for the requested records to be located e.g. your date of birth, current and previous addresses, and approximate dates of attendance. You must specify the records required and the manner in which access is sought e.g. obtain photocopies.
