Health Passports tell staff all about you and how to look after you really well.  

The Passport for Children is recommended for all children attending the South Infirmary.

 

If you have not received a Health passport yet please print one from this page.

The passports look best if they are printed in colour.

Fill in your passport and bring it to hospital with you.

Please read "How to Fill in Health Passport for Children" to help you complete the passport.

At the back of the passport there is a story. This will tell you all about what happens when you are in hospital.
